Hamilton Rating Scale for Depression (HAM-D/HDRS)

This scale helps assess the severity of depression symptoms. Please select the option that best describes how you have been feeling over the past week.

1. Depressed Mood Sadness, hopeless, helpless, worthless feelings.
2. Feelings of Guilt Self-reproach, belief of having let others down, ideas of guilt, delusions.
3. Suicide Thoughts of death, suicidal ideas, gestures, or attempts.
4. Insomnia: Early in the Night Difficulty falling asleep.
5. Insomnia: Middle of the Night Restlessness, being disturbed during the night, waking up.
6. Insomnia: Early Hours of the Morning Waking up early and difficulty returning to sleep.
7. Work and Activities Feelings of incapacity, fatigue, loss of interest in work/hobbies, decreased productivity.
8. Retardation Slowness of thought and speech, impaired ability to concentrate, decreased motor activity.
9. Agitation Restlessness, fidgeting, moving about, hand wringing, nail biting.
10. Anxiety: Psychic Subjective tension, irritability, worrying, apprehension, fears.
11. Anxiety: Somatic Physiological signs of anxiety (e.g., dry mouth, GI upset, palpitations, headaches, hyperventilation, sweating, frequent urination).
12. Somatic Symptoms: Gastro-intestinal Loss of appetite, heavy feeling in abdomen, needing laxatives.
13. Somatic Symptoms: General Heaviness in limbs, back or head; backaches, headaches, muscle aches; loss of energy and fatigability.
14. Genital Symptoms Loss of libido, menstrual disturbances.
15. Hypochondriasis Focus on bodily symptoms, preoccupation with health, complaints, requests for help.
16. Loss of Weight (Patient’s Report) Estimate of weight loss associated with the present illness.
17. Insight Acknowledgement of being depressed and ill.
