Health of the Nation Outcome Scales for Children and Adolescents. Assesses behavior, impairment, symptoms, and social functioning over the past 2 weeks.
Rating Guidance (Items 1-13): Rate the *most severe* problem during the last two weeks.
0: No problem 1: Minor problem, requires minimal action 2: Mild problem, but definitely present 3: Moderately severe problem 4: Severe to very severe problem

Rate Each Item (Past 2 Weeks)
1. Disruptive, Aggressive, Antisocial Behaviour
Verbal/physical aggression, defiance, stealing, fire-setting, vandalism.
2. Self-Injury
Deliberate self-harm (cutting, overdose, head banging – excluding suicide attempts covered in item 11).
3. Alcohol or Drug Misuse
Problems caused by misuse of alcohol, illicit drugs, solvents, or prescribed medication.
4. Problems with Scholastic or Language Skills
Difficulties with reading, writing, comprehension, expressive language affecting learning/functioning.
5. Problems with Physical Illness or Disability
Problems arising from physical illness/disability affecting mental health or functioning.
6. Problems Associated with Hallucinations, Delusions
Distress or functional impact from hallucinations, delusions, or abnormal perceptions.
7. Non-organic Somatic Problems
Physical symptoms (pain, nausea, etc.) without organic cause, linked to psychological factors.
8. Problems with Emotional and Related Symptoms
Depression, anxiety, fears, phobias, obsessions, compulsions affecting functioning.
9. Problems with Peer Relationships
Difficulties making or keeping friends, social isolation, bullying, being bullied.
10. Problems with Overactivity, Attention or Concentration
Hyperactivity, restlessness, short attention span, impulsivity affecting functioning.
11. Intentional Self-Harm with Suicidal Intent
Any self-harm rated as having definite suicidal intent (past or current).
12. Problems with Parent / Carer Relationships
Difficulties in relationship with primary caregivers (conflict, detachment, over-dependence).
13. Problems with Sibling Relationships
Difficulties in relationship with siblings living in the same household.
14. Problems with Knowledge / Understanding
Child/Adolescent’s understanding of their difficulties and treatment.
15. Problems with Information for Carers
Amount of information available to parent/carers about the condition and services.
