1. Apparent Sadness

Despondency, gloom, despair reflected in speech, expression, posture.
No sadness. 0
Looks dispirited but does brighten up without difficulty. 2
Appears sad and unhappy most of the time. 4
Looks miserable all the time. Extremely despondent. 6

2. Reported Sadness

Patient’s reports of depressed mood, low spirits, feeling beyond help.
Occasional sadness in keeping with circumstances. 0
Sad or low but brightens up without difficulty. 2
Pervasive feelings of sadness or gloominess. Mood influenced by external circumstances. 4
Continuous or unvarying sadness, misery or despondency. 6

3. Inner Tension

Ill-defined discomfort, edginess, inner turmoil, mounting panic/dread/anguish.
Placid. Only fleeting inner tension. 0
Occasional feelings of edginess and ill-defined discomfort. 2
Continuous feelings of inner tension or intermittent panic, managed with some difficulty. 4
Unrelenting dread or anguish. Overwhelming panic. 6

4. Reduced Sleep

Reduced duration or depth of sleep compared to normal pattern.
Sleeps as usual. 0
Slight difficulty falling asleep or slightly reduced/light/fitful sleep. 2
Sleep reduced or broken by at least 2 hours. 4
Less than 2 or 3 hours sleep. 6

5. Reduced Appetite

Loss of appetite, loss of desire for food, or need to force oneself to eat.
Normal or increased appetite. 0
Slightly reduced appetite. 2
No appetite. Food is tasteless. 4
Needs persuasion to eat at all. 6

6. Concentration Difficulties

Difficulty collecting thoughts, incapacitating lack of concentration.
No difficulties in concentrating. 0
Occasional difficulties in collecting one’s thoughts. 2
Difficulties concentrating and sustaining thought, reduces ability to read/converse. 4
Unable to read or converse without great difficulty. 6

7. Lassitude

Difficulty getting started, slowness in initiating and performing everyday activities.
Hardly any difficulty getting started. No sluggishness. 0
Difficulties in starting activities. 2
Difficulties starting simple routine activities, carried out with effort. 4
Complete lassitude. Unable to do anything without help. 6

8. Inability to Feel

Reduced interest, inability to react with adequate emotion, emotional paralysis.
Normal interest in surroundings and other people. 0
Reduced ability to enjoy usual interests. 2
Loss of interest in surroundings. Loss of feelings for friends/acquaintances. 4
Emotionally paralysed, inability to feel anger/grief/pleasure, complete or painful failure to feel for relatives/friends. 6

9. Pessimistic Thoughts

Thoughts of guilt, inferiority, self-reproach, sinfulness, remorse, ruin.
No pessimistic thoughts. 0
Fluctuating ideas of failure, self-reproach or self-depreciation. 2
Persistent self-accusations, definite but rational ideas of guilt/sin, increasingly pessimistic about future. 4
Delusions of ruin, remorse, or unredeemable sin; absurd and unshakable self-accusations. 6

10. Suicidal Thoughts

Feeling life is not worth living, wishing for death, suicidal thoughts/plans/preparations.
Enjoys life or takes it as it comes. 0
